Psychotherapy Services

(Please note: I’m not accepting new clients right now due to a full schedule.)

Individual Psychotherapy

I have over 30 years of experience providing competent, compassionate, respectful, short-term and long-term psychotherapy to adults. Using a whole-person, culturally sensitive approach, I help people look at their strengths and areas for growth. Clients may choose to explore the emotional, physical, and/or spiritual aspects of their therapy issues. I bring specialized insight, training and experience to help people with depression and anxiety, communication issues, sexual and physical abuse, emotional abuse and neglect, eating disorders, body image, misconduct/exploitation by other professionals, LGBTQIA+ issues, and addiction recovery.

Clients who are a good match for my services tend to:

  • Be interested in exploring and integrating whole-hearted living– mind, body, and spiritual aspects of being authentic and more connected with self and others.

  • Spend time between sessions working on therapy goals that are meaningful to them.

  • Be interested in using writing, drawing, movement, somatic experience (awareness of body signals), or other expressive modes as part of the path to healing and wholeness.

EMDR

One of many techniques that I may use in individual therapy.
Learn more about EMDR on the EMDR International Association’s public information pages.

I am trained in EMDR levels 1 and 2.

Clients may pay for services themselves and/or use their medical insurance.
I have “in-network” contracts with:

  • Blue Cross Blue Shield

  • Medica/United Healthcare/Optum

  • UCare

  • HealthPartners.

  • Medicare

  • MN Health Care Programs (Medical Assistance/Medicaid).

Virtually all insurance plans will cover my services if the plan includes out-of-network benefits or does not require you to use a specific network of providers.

Insurance

Forms

I will send new clients a link for a secure Client Portal on SimplePractice, a HIPAA-compliant medical records program that I use. You can fill out intake, Practice Policies, and insurance information online through your client portal. If you don’t have computer access, I can print and mail out these forms for you.

Cancellation Policy

Late cancellations are disappointing to everyone – you, me, and any clients on a waiting list, hoping for a session sooner rather than later. Please understand that a missed appointment or cancellation with less than 24 hours notice will be charged the full rate for the scheduled appointment.

This notice period consists of business days, not weekends or holidays, as “hours” of notice. For example, cancellations for Monday appointments must be sent the previous Friday by Monday’s appointment time.

Cancellations are accepted by phone/voicemail or email.

Payment

Payments by check or Venmo are preferred.

I accept major credit cards and HSA credit cards. You have 2 options:

  • I welcome payment by Venmo before or at the appointment.  Username: mindy@mindybenowitz.com

  • I also accept PayPal and Stripe payments.

To pay by check, make your check payable to Mindy Benowitz, Ph.D., and mail it to my office address:

Your Rights and Protections Against Surprise Medical Bills

(OMB Control Number: 0938-1401)

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate.

For questions or more information about your right to a Good Faith Estimate, visitwww.cms.gov/nosurprises.

Confidentiality as Defined by HIPPA

HIPPA is an acronym that stands for the Health Insurance Portability and Accountability Act, a US law designed to provide privacy standards to protect patients’ medical records and other health information provided to health plans, doctors, hospitals, and other health care providers. Developed by the Department of Health and Human Services, these standards provide patients with access to their medical records and more control over how their personal health information is used and disclosed. They represent a uniform, federal floor of privacy protections for consumers across the country. State laws providing additional protections to consumers are not affected by this new rule. www.HIPPA.com
